Sepulveda

To reserve the Sepulveda Resource Room for private use, you must first download, fill out, and print the Sepulveda Resource Room Contract (DOC) and Waiver Form (PDF). The completed contract and waiver form should be dropped off in the mailbox outside of the Community Center doors or mailed to UASRA 3200 Sawtelle Blvd Los Angeles 90066. Please note that you must also submit an $80 deposit with your contract and waiver form.

All residents need to reserve and turn in their forms two weeks in advance.
